What it does
EB DS 09 is the Equipment Breakdown Protection Coverage Form Renewal Endorsement. It is a declarations-series form used when an equipment breakdown policy is renewed, so the renewal can be issued by endorsement showing the renewal period and any changes to the entries on the EB DS 07 declarations. Its predecessor was BM DS 09 (07 01 edition), still listed for a few states.

Quoting and selling notes
- At renewal, compare the renewal endorsement with the prior declarations for changes in limits, deductibles, locations or equipment.
- Confirm that newly acquired locations and equipment were added.
- Jurisdictional inspection certificates for boilers and pressure vessels should be current at renewal.
