What it does

The HO DS 01 is the ISO sample declarations for all homeowners forms. It identifies the policy form (HO 00 02, 03, 04, 05, 06 or 08), the residence premises, Section I and II limits, the deductible and the forms and endorsements that make up the policy. Insurers usually reformat it, so the carrier's declarations page will not look identical.

Who needs it

Every homeowners policy. Agents use it to check underlying limits for a personal umbrella and to confirm which endorsements (home business, rental, scheduled property) are actually attached.

Quoting and selling notes

  • Compare Coverage E on the declarations with the umbrella's required underlying limit.
  • Look for HO 04 42, HO 07 01, HO 24 70 or HO 24 71 when the client has a business or rental exposure.

Editions and availability

Editions in use and where they apply: 05 11 (52 jurisdictions).

Homeowners endorsements are built for a specific program edition. When a carrier moves a state from the 2011 (05 11) program to the 2022 (03 22) program, the endorsement editions move with it, so match the endorsement edition to the policy form edition shown on the declarations.
