What it does
PR 00 05 is the occurrence version of ISO's professional liability coverage form for allied health providers such as therapists, nurses, chiropractors and similar practitioners and facilities. It covers liability for injury arising out of the rendering of or failure to render professional services by the insureds described in the form, on an occurrence basis. Its companion claims-made version is PR 00 06, and its declarations are PR DS 03.
Program context
ISO's medical professional liability (PR) program provides separate coverage forms for physicians, surgeons and dentists; hospitals; allied health care providers; blood banks; diagnostic testing laboratories; optometrists; and veterinarians, each in an occurrence version and a claims-made version, with matching declarations (PR DS 01 to PR DS 07) and shared endorsements. Forms lists show the 09 08 edition as current. ISO has filed this as its Simplified Medical Professional Liability program.
Who needs it
Allied health providers such as therapists, nurses, chiropractors and similar practitioners and facilities. The ISO program is used by carriers that write these classes on bureau forms; many medical malpractice carriers use proprietary forms instead.
Quoting and selling notes
- Occurrence: covers injury during the policy period whenever the claim is made, so no tail is needed when coverage ends.
- Occurrence forms usually cost more than first-year claims-made coverage; compare with the claims-made version.
