What it does
Form H is the uniform cargo certificate of insurance. Minnesota's rules require: "Each insurance company insuring cargo shall file a cargo certificate of insurance, form H, 'Uniform Motor Cargo Certificate of Insurance,'" described in the former federal 49 CFR part 1023 forms, or a cargo bond on Form J. It is the state counterpart of the federal BMC-34.
Who needs it
Carriers operating under state authority where the state requires cargo security, commonly household goods movers and some general commodity carriers, depending on the state.
Key rules
- The insurer files the certificate with the state commission.
- Cancellation is made on Form K, the same uniform cancellation notice used for Form E, by checking the cargo box.
Selling notes
Check each state's required cargo limits and whether the carrier's motor truck cargo policy meets them. Tow operators and other specialized carriers sometimes ask whether on-hook coverage satisfies a state's cargo requirement; the state regulator decides, so get its answer in writing.
