What it does
Form F is the uniform motor carrier liability insurance endorsement attached to the carrier's auto policy, the policy-side companion to the Form E certificate filed with the state. Washington's rule for passenger transportation companies, for example, states: "The insurance policy must include the Uniform Motor Carrier Bodily Injury and Property Damage Liability Endorsement (Form F)," and separately requires a Form E filing with the commission.
Who needs it
Intrastate carriers in states whose rules require the endorsement, typically alongside a Form E filing. Texas uses its own version for intrastate carriers; ISO publishes a Texas Form F-1 endorsement (CA 23 36).
Key points
- Like the federal MCS-90, Form F is an endorsement on the policy itself that supports a regulatory filing.
- Cancellation is coordinated with the state filing; the Form E stays in effect until a Form K notice is filed.
Selling notes
Ask the insurer to confirm it will issue the Form F and file the Form E in each state where the carrier holds intrastate authority. Read the endorsement for any reimbursement obligation the carrier takes on.
