What it does
EP DS 01 is the declarations form for ISO's Employment-Related Practices Liability coverage part, used with EP 00 01 or EP 00 02. It shows the named insured, policy period, limits of insurance, deductible or co-payment selections and the dates that control which claims and wrongful acts the policy will answer for. Carrier filings that adopt the ISO EPL program list EP DS 01 09 07 with EP 00 01 11 09.

Quoting and selling notes
- Check the limit, retention and any co-payment against the quote.
- When replacing an existing EPL policy, confirm any retroactive or prior-litigation date shown; a later date can leave a gap.
- Some carriers use their own supplemental declarations alongside it.
