What it does

The 06 22 forms define both "ERISA employee benefit plans" and "Non-ERISA employee benefit plans" (welfare or pension plans not subject to ERISA). CR 25 51 schedules a limit for non-ERISA plans covered under the policy.

Who needs it

Churches, governmental bodies and other sponsors of plans exempt from ERISA.

How it fits the policy

This endorsement changes the base crime or fidelity form it is attached to and is listed with the other endorsements on the declarations. Schedules in the endorsement (names, positions, amounts or dates) control how it applies, so confirm the schedule is complete before binding. ISO's forms index lists the current edition for 52 jurisdictions.

Quoting and selling notes

  • Confirm the plan's ERISA status with the sponsor or its counsel.
